CLE Forms

Sponsor application forms to apply for CLE accreditation are no longer available on the website. Sponsors must apply for accreditation of their CLE activities through the Sponsor Portal. If you are a county agency or municipality who cannot pay the application fee by credit card or ACH payment, please email CCLE@sc.ohio.gov to request an application form. Attorneys may apply for CLE accreditation for live in-person, out-of-state activities. Application forms should be emailed to OHCLEapp@sc.ohio.gov. Pursuant to Reg. 409, attorneys and judges cannot apply for accreditation for self-study activities. Self-study courses are identified as activities held outside of the standard classroom setting, including formats such as live webinars, on-demand, and telephone activities.
