Domestic Relations and Juvenile Standardized Forms
The Supreme Court of Ohio developed standardized forms for termination of marriage, child support, and custody matters in domestic relations and juvenile courts to assist self-represented litigants access the justice system. These forms were originally adopted in 2010 pursuant to Ohio Rule of Civil Procedure 84 and Ohio Juvenile Rule of Procedure 46. The most recent amendments to these forms became effective June 2021.
Although these forms are provided by the Supreme Court of Ohio, they should be filed in the local county court. Please note that the local court may have additional forms that must be filed. Click here to find your local court.
The PDF files may be downloaded to your computer. It is strongly recommended that you download the latest version of Acrobat Reader.
Microsoft Word versions of forms are available upon request to courts or legal aid organizations by contacting cfc@sc.ohio.gov.
